We are happy to introduce our new secure Patient Portal.  This addition will allow patients to communicate securely with our office to ensure privacy.  The portal is a secure, encrypted, self-contained website.  By creating a user account and logging into the portal, patients will have access to the following features from the convenience of any internet-connected computer, 24 hours a day, 7 days a week:

• Appointment Request
• Prescription Refill Request
• Update Personal Information (address, phone, insurance, etc.)
• Billing Questions
• General Questions
• Non-urgent Medical Questions
• Access Online Forms
  • New Patient Registration
  • Health History Questionnaire
  • Followup Visit Form
